User Interface in a Toilet

Association Coffee in Creechurch Lane, London EC3, is a coffee shop so good it couldn't possibly be in the City. Calm atmosphere, places to sit, good magazines, jugs of water, good soup and great baking. And fabulous coffee.

My final requirement is a toilet, and theirs is great - spacious and decorated in a coolly distressed manner. Unfortunately, like so many cool user interfaces, it turns out it doesn't quite work, and has had to be "improved" by documentation:
